To cancel or reschedule a lesson, a 48-hr notice is needed. Canceling with less than 48 hrs, you will be charged for a fee of $35. Canceling after the lesson's start time or without notice will be charged for the full price. Lessons are non-refundable.*

AGREEMENT OF RELEASE & WAIVER OF LIABILITY

1. I am participating in the Dance Lessons or Parties offered by Green Bay Ballroom LLC ("dance Studio") during which I will receive information and instruction about dancing. I recognize that dancing requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Dance Lessons or Parties. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the Dance Lessons or Parties.

3. In consideration of being permitted to participate in the Dance Lessons or Parties, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might incur as a result of participating in the program.

4. I understand that Green Bay Ballroom LLC is not responsible for personal property that is lost, damaged or stolen while I am at Green Bay Ballroom LLC property.

6. In further consideration of being permitted to participate in the Dance Lessons or Parties, I knowingly, voluntarily and expressly waive any claim I may have against Green Bay Ballroom LLC and/or its agents for injury or damages that I may sustain as a result of participating in the program.

7. I authorize and agree that Green Bay Ballroom LLC may take and use photographs, videos or likenesses of myself as needed for its record-keeping, advertising and/or public relations projects and that I have no rights to the same and will not be compensated for the same. I understand that I must notify Green Bay Ballroom LLC prior to participation in class if I do not wish to comply with this policy.

8. I understand that recording of the Dance Lessons or Parties is not permitted without instructor’s consent. I agree to not reproduce, sell, share or distribute any recordings of the Dance Lessons or Parties.

9. I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue Green Bay Ballroom LLC and/or its agents for any injury caused by their negligence or other acts.
